Sinergia Inc. Residential Habilitation Specialist in New york, New York
General Responsibilities:
To enhance the well-being of individuals and families through the human service delivery system(s), with particular concentration on those special populations which have traditionally experienced difficulty in accessing human services.
FUNCTIONAL SKILLS TRAINING SERVICES- will assist in developing skills of personal hygiene and grooming, selection and/or preparation of food, cleaning and washing of clothes, maintenance of the personal environment, budgeting and money management, education in general health, family planning and use of community resources such as public transportation and recreation.
a)Health & Wellbeing- will assist individuals in administration of medication administration, work in coordination with our team members, including service coordinators, nurse to help people access required medical, mental health and other related services that promote their health and wellbeing.
b)Assertiveness/Self-Advocacy Skills Training Services- will promote the persons ability to use strengths and capacities to make life changes, choices and to increase self-awareness about personal values and preferences to increase responsiveness to medical, safety, and other personal problems, to improved communication skills and facilitate appropriate interpersonal behavior.
c)Behavior Skills Training Services- will facilitate development of skills and capacities (or limit regression) in functional areas, such as interpersonal behavior and appropriate social behaviors in various community settings.
d)Communication Skills Training Services- will facilitate consumers development of skills and capacities in functional areas, such as expressive language, listening skills, adaptive/special communication devices.
e)Community Integration and Resource Utilization Skills Training Services- will facilitate consumers abilities to identify and learn to access community services and supports, to assess consumers existing natural and community supports and to increase networks and support systems in the community integration.
f)Documentation- Residential habilitation staff will complete proper records and maintain documentation, participate in residential habilitation plans to ensure the implementation of services and supports to help individuals achieve their goals and life plans
g)Incident reports- staff will report any incidents that affect the wellbeing or violate consumer rights.
h)Fire Drills- staff will conduct and document fire drills and evacuation procedures.
i)Will escort and accompany consumers to appointments as needed.
j)Will participate in training provided by Director of Development and other as may be assigned.
k)Will observe ethical code of conduct and respect individuals whom they serve.
Professional Conduct:
Serves as a role model for all Agency staff in promoting the Agency Mission, Vision, Values, and Guiding Principles. Fosters agency and program wide cooperation and teamwork through use of positive/constructive communication techniques. Maintains absolute confidentiality in regards to all people served and personnel records.
Skills:
Strong organization and record keeping skills Excellent communication, both orally and in writing Proficient computer literacy Strong presentation skills Knowledge of communication techniques with individuals with an intellectual/developmental disability Experience working with individuals with intellectual and developmental disabilities Advocacy skills
Physical Demands:
Ability to lift at least 50 pounds or more Daily standing, bending and lifting Must be able to stand for at least eight hours at a time
Educational Requirements:
High School Education Preferred / GED
