Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We are two of New York City’s leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults through-out Manhattan and New York City. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational.

 

Program Description: The West Side NORC (Naturally Occurring Retirement Communities) promotes the concept of “aging in place” by providing a myriad of services for residents which enable them to remain in their homes and lead healthy and fulfilling lives. The Goddard Riverside West Side NORC is comprised of five cluster buildings on the upper west side and serves residents age 60+. Comprehensive services include social work/case assistance/management services, health promotion services, recreational/educational/social group activities, as well as other support services provided by on-site Goddard Riverside staff.

 

Position Summary:The NORC Case Manager provides individualized services to enable seniors to live independently. The Social Worker works with residents, their families, and support systems to access any additional services that promote healthy, safe and meaningful lives. Specific services include: assistance with applications and recertifications for benefits, including Medicare, Medicaid, long term care, SNAP, HEAP, accessing Food Resources, referrals and providing connections to medical care, in-home care, legal services, collaboration with the NORC VNS Nurse, and coordination with NORC case assistance services such as chore assistance, escort services, home visits, telephone reassurance calls. Additional services include helping NORC members access educational, cultural and recreational activities that provide a sense of value, well-being and community.

 

Schedule:Monday-Friday (35 hours per week, some evening work)

 

Salary Range:  $45,742 - $52,000

 

Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.

Roles, Responsibilities, and Essential Duties

Role, Responsibilities, and Essential Duties

Client Services:

  • Provide Individualized case management services to NORC members, ensuring that the needs of individuals and the community are met

  • Support the functioning of all services within the site and collaborate with NORC Case

  • Assistance staff members, Nurse, Volunteers, Consultants and Program Director

  • Provide emergency assistance and outreach if needed

  • Develop outreach strategies and outreach activities to engage and enroll new participants

  • Ensure a culturally responsive, safe, inclusive, respectful and welcoming environment.

  • Conduct ongoing outreach and collaboration with other agencies to access additional resources and services

  • Communicate and coordinate with NORC building management and staff

     

    Program Development:

  • Collaborate with Program Director to develop additional NORC services based on member need or interest

  • Collaborate and build sustainable partnerships

  • Attend community meetings to cultivate relationships with community members, other services providers, local government entities

  • Assist with the coordination and supervision of volunteers and consultants

  • Participate in and help to develop special projects or new initiatives

     

    Program Administration:

  • Implement, support and follow all agency policies and procedures.

  • Complete all chart work and requirements in a timely and thorough manner

  • Ensure readiness for audits by funders, in-house or as needed

  • Establish and enforce safety rules and emergency procedures.

  • Attend staff meetings, agency-wide meetings and trainings, supervision sessions

  • Participate in program assessment and improvement plans

  • Other duties as required and assigned by Director

     

    Qualifications/Educational Requirements:

  • Minimum of Bachelor’s degree with case management experience with older adults

  • Experience working in a professional environment with older adults

  • Fluency in a second language is a plus (Spanish preferred)

     

    Skills, Knowledge and Abilities:

  • Strong clinical, writing and administrative skills

  • Knowledge of the strengths and needs of older adults

  • Ability to communicate with and engage members who may present with some barriers (physical, hearing or visual impairments, mental health issues, etc.)

  • Ability to multi-task and work in a fast-paced environment

  • Ability to write accurate and concise chart notes

     

    Computer Skills:

  • Experience with Microsoft Office programs, especially Outlook, Word and Excel

  • Ability to learn and use electronic databases

     

    Physical Requirements:

  • Ability to walk and stand frequently, make home visits to the various NORC buildings

  • Ability to travel to homebound members and hospitals, rehabs if needed

     

    Work Environment:

  • Shared Office spaces and Community Rooms in several buildings

  • Occasional home visits, hospital and rehab visits, or escort to appointments

  • Remote work if necessary
