SUMMARY:

The COMPASS Case Manager works as part of an interdisciplinary team to support the overall care and treatment of patients living with HIV/AIDS by addressing their case management needs.

RESPONSIBILITIES:

  • Provides concrete case management regarding a variety of needs, including but not limited to: HIV/AIDS Service Administration (HASA), AIDS Drugs Assistance Program (ADAP), insurance enrollment, transportation assistance, coordination with pharmacies, housing referrals, completion of M11Q forms.

  • Assesses patients medication adherence, and provides health education as needed to support good adherence practices

  • Provides appointment reminder and outreach activities as assigned.

  • Provides health promotion, treatment adherence counseling, and risk reduction counseling to assist patients in decreasing barriers to viral load suppression and retention in care.

  • Engages patients to screen for program eligibility, complete needed intakes and reassessments as dictated by program guidelines

  • Completes individualized service plans with service recipients to address case management needs.

  • Participates in case conferences with other team members to ensure patient’s psychosocial needs are being addressed.

  • Completes needed screenings per eHIVQUAL recommendations, such as depression, anxiety and substance use screenings and refers patients as appropriate.

  • Participates in Continuous Quality Improvement (CQI) projects, to support the overall health of patients as well as viral load suppression and retention in care

  • Works closely with medical providers, nursing staff and other Institute employees to coordinate care for patients, through formal and informal case conferences, huddles, warm hand-offs and joint visits

Qualifications

  • Bachelor's Degree in social work or related field required

  • Computer literacy with proficiency in MS Word, Excel, and PowerPoint.

  • Demonstrated organizational, interpersonal, oral and written communication skills and the ability to handle multiple assignments at any time

  • Ability to generate, interpret and analyze data from multiple sources

  • Knowledge of social services, particular for individuals with HIV, in the NYC area

  • Bilingual English/Spanish preferred.
