R.A.I.N. Total Care Inc. PACT Case Coordinator in Bronx, New York

Position Summary

Determine client needs (residents and families: children, youth, adults and older adults) and eligibility for assistance, arrange for services, coordinate activities and programming. Work under the guidance of the Program Director

Essential Duties and Responsibilities Conduct outreach, maintain a caseload, create surveys and collect input from residents for program development. Provide direct services to residents, including intake, referrals, benefits and entitlements assistance, advocacy, counseling and follow-up. Provide crisis intervention when needed. Maintain and update residents files and ensure that all information is documented, and all contractually required documents are maintained in files and database. Fill out entitlements/benefits applications such as Supplemental Nutrition Assistance Program (SNAP), Medicaid, Medicare Savings Program, Senior Citizen Rent Increase Exemption (SCRIE), Home Energy Assistance Program. Assist with resident move process. Provide referrals for after school programs, tutoring, youth programs and childrens services. Maintain compliance with case assistance and referral standards. Arrange for provision of services, advocate with other agencies on behalf of residents. Responsible for the submission of case assistance and program logs and statistical reporting weekly and enter database. Create monthly programming calendars with programming and schedule activities, presentations and events. Collaborate with community-based organizations and other Case Coordinators within the program for partnerships and resources. Attend In-service trainings. Perform additional duties as assigned by supervisor

Minimum Qualifications Associates degree, or higher with two years of supervised experience in human services Knowledge of benefits and entitlements for older adults. May to October from 2 pm-7pm
