SVP Pharmacy Corp., d/b/a Sinai RX Pharmacy Health Educator- Supervising coordinator in Bronx, New York
The Health Educator position is responsible for providing medication therapy management on eligible patients, comprehensive medication review, patient education, monitoring, physician consultations, patient compliance consultations and targeted intervention. The position will also encourage vaccinations, establish relationships with local doctors and community, work with facility personnel to establish care strategies for chronic diseases that are prevalent and educate staff on new methods of approach to chronic disease management. Responsibilities include:1.Plan, develop and design health education and training programs, including preparation, facilitation, and follow-up.2.Evaluate programs; perform research on relevant topics and develop and/or revise curriculum modules and/or presentation materials as appropriate.3.Present lectures and training to groups and/or individuals, as appropriate.4.Coordinate the activities of consultants, agencies, and other providers involved in specific programs.5.Participate in the development of marketing and promotional goals and strategies, as appropriate;6.Serve as a point of contact in area of expertise; refer clients to other support services, such as medical evaluation and treatment, social services, and employment services, as appropriate.7.Prepare and disseminate educational and informational materials.8.Maintain current knowledge base on specific health issues, particularly in area of expertise.9.Perform miscellaneous job-related duties as assigned.Position Requirements: Master’s Degree in Public Health, Health Promotion, Health Education, Integrative Wellness or related field (Nursing, Public Administration/Public Health). 24 months experience as a Health Educator or Health Education Specialist.Position Hours: Flex schedule - M-F / 9am-5pm - unless otherwise arranged.Qualified candidates should submit resume with salary requirements via mail to Sinai Pharmacy - Attn: HR, 903B Sheridan Avenue, Bronx NY 10454.
